Support Coordinator – Fixed term 1 year
We are recruiting a Support Coordinator to develop and implement the phased roll-out of the OPRAH (Older People Remaining at Home) programme.
This is a research action project aimed at supporting older adults to remain living in their own homes in conjunction with Ageing Well Network. The Support Co-ordinator will work within the multi-agency structure of the Age Friendly City and Counties programme to identify and create individually tailored support packages reflecting the physical, psychological and social needs of older adults.
